  1. Establish an account
    1. Fill out the REDCap request form
      1. REDCap Request Form
    2. A completed "Statement of Liability", DocuSign attestation form, is required of each REDCap user and must be renewed every 2 years. The form will be automatically sent when a REDCap account is requested.
    3. Access for External Collaborators
      1. Registering external collaborators as affiliates through HR is encouraged. Utilize the HR SRS system available through to register your external collaborators.
      2. It is also possible to request access for collaborators without registering them as affiliates through a service now request.
    4. If you already have a REDCap user account, REDCap can be accessed by visiting
