- Establish an account
- Fill out the REDCap request form
- REDCap Request Form
- A completed "Statement of Liability", DocuSign attestation form, is required of each REDCap user and must be renewed every 2 years. The form will be automatically sent when a REDCap account is requested.
- Access for External Collaborators
- Registering external collaborators as affiliates through HR is encouraged. Utilize the HR SRS system available through myaccess.ucsf.edu to register your external collaborators.
- It is also possible to request access for collaborators without registering them as affiliates through a service now request.
